Simply follow these steps to start the process.

How Does the Dementia Care Navigation Process Work?

Step 1: Free 30-Minute Introductory Chat

Let’s Talk
You’ll meet with Mary Caldwell, our experienced gerontologist, for a no-cost, no-pressure 30-minute call. This is your chance to share what’s going on and explore how we can help.

Step 2: In-Depth Consultation

Let’s Dive Deeper
If you decide to move forward, we’ll schedule a comprehensive care consultation—your choice of in-person or virtual. During this time, we’ll take a deeper look at your situation and begin crafting a personalized path forward.

Step 3: Receive Your Custom Care Plan

Following the consultation, you’ll receive a detailed, written Care Plan that:

  • Addresses individual needs and requirements
  • Clearly defines care goals

FAQ’s

How Does Dementia Care Navigation Support Caregivers?

Caring for someone with dementia can be a rollercoaster—emotional, unpredictable, and sometimes overwhelming. That’s where we come in. Skylark Care Navigation offers personalized consultations and care plans that put you and your loved one at the center. With coaching, resources, and expert guidance, we help you feel more confident, connected, and in control. The result? Less stress, more structure, and a greater sense of support on your caregiving journey.

How Does DCN Benefit People Living with Dementia?

Our goal is simple: help your loved one live well with dementia as independently as possible for as long as possible. With nearly two decades of experience in dementia care, we focus on helping people stay independent, engaged, and purposeful. Whether it’s through meaningful activities or small daily routines, we aim to bring joy and dignity into every stage of the journey—strengthening the bond between you and your loved one along the way.

Does Dementia Care Navigation Save Money or Offer Practical Benefits?

Absolutely! Think of us as your care compass. We help you navigate the maze of dementia-related challenges—medical, financial, legal, and emotional. From Medicaid to VA benefits to local programs you may not even know exist, we help you unlock support systems that can ease your burden and protect your resources. Our care plans grow and adapt right alongside your loved one’s needs.

Can DCN Help Avoid or Delay a Move to a Nursing Home?

Yes—we work with families to create a plan that aligns with your goals, whether that’s staying at home or preparing for a future care setting. We’ll help you know when it’s time to consider in-home help, adult day programs, or other options. With clear goals and expert guidance, you can make transitions with confidence—not crisis.

How Does DCN Help with the Loneliness of Caregiving?

Caregiving can be isolating—but you don’t have to do it alone. DCN creates a bridge between families, healthcare professionals, and support networks. We’ll help you build a care team that truly “gets it,” so you’re surrounded by people who understand what you’re going through and are ready to help.

Is Ongoing Support Included?

Yes! Whether you want a one-time plan or ongoing guidance, we’ve got you. You can opt in for regular check-ins, plan updates, or just reach out when something changes. Think of us as your on-call dementia care team, ready to jump in when you need us most.

Who Are the People Behind DCN?

Our DCN program is led by Mary Caldwell, a compassionate gerontologist with a Master’s in Aging Studies and a background in social work. She’s worked with two Area Agencies on Aging and the Alzheimer’s Association—so she brings real-world knowledge, heart, and dedication to every family. Alongside a team of experienced nurses and dementia care pros, she ensures you get ethical, expert, and affordable support.

We Don’t Have a Diagnosis Yet—Can You Still Help?

Yes, and it’s a great idea to start early. Dementia is a broad term, and getting the right diagnosis—whether it’s Alzheimer’s, Vascular Dementia, or something else—is crucial. We’ll guide you through the process and connect you with trusted neurologists so you get answers and a clear path forward.

Can You Help During a Crisis?

Life doesn’t always wait for a plan—and we get that. If you’re facing a sudden challenge or unexpected behavior change, we’re here to help. Our team can step in with calm, experienced support when you need it most—helping you navigate difficult moments with confidence and care.
At Skylark, we believe no one should face dementia alone. Whether you’re just starting to notice changes or are deep into the caregiving journey, we’re here to walk beside you every step of the way.
